The following benefit information is a summary of the 2010 benefits for full-time salaried and eligible part-time salaried employees. Please refer to Agency Policies and Summary Plan Descriptions for additional details. Health Insurance — Blue Cross & Blue Shield, Inc. The Family Conservancy offers employees and their family members a choice of two plans. Full-time employees choosing the “Base Plan” POS 80/50 plan will have their health insurance premium paid 100% by the Agency. Employees choosing the “Buy-up Plan” POS 100/80 plan will pay the premium difference, with The Family Conservancy contributing toward the premium at the “Base Plan” POS rate. Please contact the Human Resources department for additional premium information. Dental Insurance — Principal The Family Conservancy offers a choice of two voluntary Dental PPO plans for employees and their family members. Employees pay a nominal monthly premium. Vision Insurance — VSP-Vision Service Plan The Family Conservancy offers vision insurance for employees and their family members. The Agency contributes toward the cost of the nominal monthly premium for enrolled employees. VSP coverage allows employees an annual eye examination and a choice between glasses or contacts yearly if medically prescribed. Life Insurance — Reliance Standard, Inc. Life insurance benefit at twice the annual salary, with premiums paid by the Agency. Accidental Death and Dismemberment — Reliance Standard, Inc. Agency paid benefit for employees who are permanently disabled, at twice the annual salary. Long Term Disability — Reliance Standard, Inc. Agency paid benefit for employees who are unable to work due to a long-term illness or injury, 90 day elimination period. Benefit begins on the 91st day and pays 60% of monthly salary. 403b Thrift Plan — Mutual of America Agency paid retirement benefit. Eligible staff receive agency contributions equaling 5% of their gross salary, plus an additional 2% match for employee contributions upt to 2%. Employees are eligible to voluntarily contribute to the plan on the first of the month following date of hire. Agency contributions begin after one year of service duing which employees must work a minimum of 1,000 hours in a continuous 12-month period. Refer to plan SPD for additional information. FSA-Flexible Spending Account (Cafeteria Plan) — Paychex, Inc. Agency paid benefit, administered by Paychex, which allows employees to contribute toward pre-tax savings for medical reimbursement, dependent care, and medical insurance premiums. Annual Leave Vacation benefits for employees are accrued monthly according to length of service. Accrual rates: - 1-12 months of service 6.25 hrs/month = 2 weeks/yr
- 13-60 months of service 9.375 hrs/month = 3 weeks/yr
- 61+ months of service 12.5 hrs/month = 4 weeks/yr
Sick Leave Sick Leave benefits accrue monthly according to length of service. Accrual rates: - 1-12 months of service 7.5 hrs/month 2.4 weeks/yr
- 13+ months of service 9.0 hrs/month 2.8 weeks/yr
Sick Leave Pool Financial assistance program which grants additional sick leave to employees who are unable to work, who have exhausted all other forms of paid leave, and who will likely suffer loss in compensation because of an extended illness or injury. The Sick Leave Pool balance is maintained through voluntary contributions of sick leave donated by employees. Personal Days The Family Conservancy grants two personal days to employees each year, awarded on January 1st.
